For this assessment you will create a 2-4 page plan proposal for an interprofessional team to collaborate and work toward driving improvements in the organizational issue you identified in the second assessment.

The health care industry is always striving to improve patient outcomes and attain organizational goals. Nurses can play a critical role in achieving these goals; one way to encourage nurse participation in larger organizational efforts is to create a shared vision and team goals (Mulvale et al., 2016). Participation in interdisciplinary teams can also offer nurses opportunities to share their expertise and leadership skills, fostering a sense of ownership and collegiality.

You are encouraged to complete the Budgeting for Nurses activity before you develop the plan proposal. The activity consists of seven questions that will allow you the opportunity to check your knowledge of budgeting basics and as well as the value of financial resource management. The information gained from completing this formative will promote success with the Interdisciplinary Plan Proposal. Completing this activity also demonstrates your engagement in the course, requires just a few minutes of your time, and is not graded.

Professional Context

This assessment will allow you to describe a plan proposal that includes an analysis of best practices of interprofessional collaboration, change theory, leadership strategies, and organizational resources with a financial budget that can be used to solve the problem identified through the interview you conducted in the prior assessment.

Scenario

Having reviewed the information gleaned from your professional interview and identified the issue, you will determine and present an objective for an interdisciplinary intervention to address the issue.

Note: You will not be expected to implement the plan during this course. However, the plan should be evidence-based and realistic within the context of the issue and your interviewee's organization.

Instructions

For this assessment, use the context of the organization where you conducted your interview to develop a viable plan for an interdisciplinary team to address the issue you identified. Define a specific patient or organizational outcome or objective based on the information gathered in your interview.

The goal of this assessment is to clearly lay out the improvement objective for your planned interdisciplinary intervention of the issue you identified. Additionally, be sure to further build on the leadership, change, and collaboration research you completed in the previous assessment. Look for specific, real-world ways in which those strategies and best practices could be applied to encourage buy-in for the plan or facilitate the implementation of the plan for the best possible outcome.

Using the Interdisciplinary Plan Proposal Template [DOCX] will help you stay organized and concise. As you complete each section of the template, make sure you apply APA format to in-text citations for the evidence and best practices that inform your plan, as well as the reference list at the end.

Number of references: Cite a minimum of 3 sources of scholarly or professional evidence that support your central ideas. Resources should be no more than 5 years old.
APA formatting: Make sure that in-text citations and reference list follow current APA style.

The main issues facing Hialeah Hospital are communication and collaboration issues among various healthcare professionals creating vulnerabilities and hindering them from offering effective healthcare. The plan to overcome the challenge facing teams will be pursued within the healthcare environment to reduce workstyle and personality conflict between healthcare professionals.

Objective

To implement an evidence-based approach that integrates team-building activities and ensures healthcare roles are linked to promoting collaborative culture among healthcare specialists. This objective aligns with healthcare’s organizational goals of proactively creating opportunities for different healthcare specialists to participate in decision-making (Stahlke & Dahlke, 2020). In addition, developing an interdependence solution to resolve team issues at the healthcare center will foster cohesion between teams improving the quality of care and healthcare outcomes for patients (Meyer et al., 2014).

Questions and Predictions

  1. How will the evidence-based approach improve cohesion among the team?

The healthcare organization has not fully implemented any evidence-based approach to creating team interdependence. However, by adopting evidence-based practices such as change theory, leaders can proactively involve their employees in creating opportunities for communication and collaboration between different practitioners (Meyer et al., 2014). Effective communication will not only boost healthcare outcomes but will also increase knowledge sharing among colleagues.

  1. Will Lewin’s model effectively create a harmonious environment for team members and employees?

There is significant evidence that Lewin’s change management model has positively contributed to effecting the desired organizational changes. However, for the model to be effective, necessary actions from leadership and management are required to motivate employees and affect the change process (Hussain et al., 2018). Similarly, leadership will need to monitor, evaluate and plan changes continuously based on how the internal and external environment responds.

  1. Will the collaborative leadership techniques be effective in creating interdisciplinary solutions?

The leadership strategy aims to adopt collaborative techniques to develop disciplinary solutions. Thus, by necessitating the leadership structure to adopt a change process, leadership will proactively involve their employees in creating opportunities that emphasize problem-solving, collaborative decision-making, and knowledge sharing among different health practitioners (Meyer et al., 2014). 

Change Theories and Leadership Strategies

Lewin’s change theory has the greatest potential for creating an organizational solution that will fulfill the internal and external requirements of Hialeah Hospital. For positive change to be experienced, the organization must understand why change must be adopted following the three distinct stages of Lewin’s model: unfreezing, changing, and refreezing (Hussain et al., 2018). The different healthcare professionals will be notified of the Hialeah Hospital’s interdisciplinary challenges during unfreezing. Leadership will emphasize why the changes are necessary using tangible factors such as healthcare outcome surveys and patient feedback surveys to show them the severity of the situation. In the change phase, the leadership and management of Hialeah Hospital will attempt to resolve the healthcare challenges by proactively involving employees in meetings and discussions as it creates opportunities for employees to air their opinion and participate in the decision-making process. Communication will be vital during the planning and implementation of change since it will require the leadership to get all practitioners on board by clearly explaining how the changes will affect them, their patients, and the entire healthcare ecosystem (Stahlke & Dahlke, 2020). This stage will be an ongoing process whereby employees will be empowered by leadership through positive reinforcement strategies. The final stage of change theory, refreezing, will take shape when healthcare practitioners at Hialeah Hospital embrace the changes and create a harmonious environment for team members and patients. To anchor part of Hialeah Hospital’s culture, leadership will establish a feedback system and offer support that reinforces teamwork and collaboration (Hussain et al., 2018).

As part of the transformational leadership strategy in Lewin’s model, Hialeah Hospital will foster collaborative practices across its healthcare teams. Leadership will identify the requested changes from staff and collaborate with staff to improve their healthcare team’s effectiveness by identifying their strengths and weaknesses (Hussain et al., 2018). Collaborative leadership techniques aim to develop effective teams by ensuring effective communication and collaboration through decision-making.

Team Collaboration Strategy

  • Hialeah Hospital’s governing board will give the go-ahead for the interdisciplinary solution.
  • The hospital’s director general will offer the appropriate resources required to facilitate the widespread implementation of interdependence solutions to resolve the team issues at Hialeah Hospital.
  • Each department manager will proactively lead their staff in meetings and discussions to create opportunities for employees to express their opinion and participate in the decision-making process.
  • The case manager will supervise the implementation of evidence-based approaches across interdisciplinary teams while the team members will collaborate on problem-solving, decision-making approaches, and knowledge sharing.

Collaboration in Hialeah Hospital will mostly be interdisciplinary, requiring team members to trust each other, share knowledge and resolve health issues by communicating effectively (Mach et al., 2021). These measures will not only to improves patient care and outcomes but it increases their job satisfaction and maximizes their organizational efficiency

Required Organizational Resources

A variety of resources will be required to accomplish the planned proposal. Foremost, the Hialeah Hospital will require to update its database system to enable practitioners to easily retrieve information on different patients. The database management system will be purchased for $7,000 (MocDoc, 2020). They will also require mobile monitors for each practitioner to enhance effective communication regarding patient needs. The total cost of the mobile monitors will be $4,000. The healthcare staff will also require to be trained on using the database management system and the mobile monitors by IT specialists at $5,000. The hospital will also hire a case manager for three months to ensure that the teams implement the evidence-based approaches while fulfilling their daily roles. The case manager’s salary will be $30,000 for the three months (Writers, 2021).
